Lactation Care Management in Urban and Rural Primary Care Settings: Ensuring Continuity of Care for Breastfeeding and Chestfeeding Families.

Provider: University at Albany School of Public Health
Description: Gaps exist in the continuity of care for breastfeeding and chestfeeding families, particularly in primary care settings. This webinar will review these gaps and their root causes and explore the role of a community lactation coordinator to assist pregnant and postpartum families in navigating and accessing, in a timely manner, appropriate community services that primarily serve families experiencing the greatest breastfeeding and chestfeeding inequities in the community.  Two health care practices will share information from their experiences implementing lactation care management programs in rural and urban settings, and how the programs can be an effective approach to reduce breastfeeding and chestfeeding disparities. Participants will have an opportunity to discuss how communities can adopt and implement a lactation care coordinator role to support families with their breastfeeding and chestfeeding goals.
